Joining Our Practice
The doctors welcome new patients. We do not refuse registration on the grounds of a patient being unable to provide proof of identity, address or evidence of immigration status.
Any patient requesting to register with the Practice (over the age of 5 years) will be requested to undertake a medical examination before registration is completed.
Patients will be registered with the Practice, not an individual Doctor. As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor. If patients have repeat medication they require an initial appointment must be made with the GP for this to be added to their record at the Surgery.
Medical treatment is available from the date of registration. Please contact reception for further information.
Horden Group Practice has a Zero Tolerance policy and patients will be asked to sign a form when registering.

Temporary Patients
We are always happy to see any relatives or friends staying with you if they need a Doctor or Nurse. They will need to fill out a short form for the information to be returned to their own Doctor.
Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms.
Chaperones
At the Practice we always aim to treat patients in a comfortable and safe environment with dignity and respect, to ensure that you receive the best possible care.
All patients are entitled to have a chaperone present for any consultation if they wish, this could be for a discussion, examination or procedure if they feel this is necessary.
The chaperone may be a family member or friend or it can be a member of the clinical staff or administration staff.
